Who is Eligible to Join ArchCare Community Life?

To participate in ArchCare Community Life Managed Long-Term Care Plan, you must meet all of the following requirements:

  • 21 years of age or older
  • Receiving or eligible for Medicaid
  • Require more than 120 days of community-based long-term care services
  • Reside in our current service area, which currently includes Manhattan, The Bronx, Staten Island, Queens, Brooklyn, Putnam and Westchester counties
  • Able to live safely in the community when you enroll
  • Have a nursing assessment to determine eligibility
